Spousal Benefits
This $3,000 benefit will assist you if your spouse dies while you are an active employee and legally married. Benefits will be paid in a lump sum to the employee, only upon receipt of the Death Certificate. Please contact the Fund Office.

Dependent Life Benefit
This lump sum benefit of $3,000.00 (effective 5/1/03) will assist you should your legal dependent die while you are an active employee. Benefits will be paid to the employee, only upon receipt of Birth Certificate and Death Certificate. Please contact the Fund Office.
